PROCEDURES FOR PASSPORT APPLICATION
1. Birth Certificate: A certified copy from the State where born with parents' names, showing filing & birth dates, and a seal (raised or embossed) must accompany the application. A hospital certificate is not acceptable. A previous passport will also suffice if you are reapplying.
2. Passport Photos*: Two passport photos (with an all white background), usually taken by a professional, must accompany the application. Members of AAA often take advantage of the less expensive photos sold by that organization.
3. Driver's License: A driver's license must be produced during the application process. Applicants 16 or 17 years old with a driver's license may apply with one parent present. Applicants younger than 16 years old must be accompanied by both parents.
4. Application Form (DS-11): Must be filled-in completely using black ink except for the signature. DO NOT sign the application! It must be signed in front of the Clerk.
5. Costs:
- Checks and money orders must be payable to: U.S. Department of State.
- For persons under 16 years old, the cost is $80.00 for a 5-year passport.
- For persons 16 years or older, the cost is $110.00 for a 10-year passport.
- No cash accepted for the above fees.
- All applicants must pay a separate $25.00 Clerk's fee (cash or check payable to Clerk of Courts). An additional $1.00 will be added if you do not bring a photocopy of your drivers license.
6. Most applications must be made in person at any of the following locations:

*Two extra passport photos are required by the Campus Director for use overseas in case your passport is lost or stolen. It would be wise to have two sets of passport pictures taken at this time: two for your passport application and two for the program. This applies to all travelers.
